The decision is made by the child, parents and medical professionals
They don’t rush off and chop off the child’s genitals

First the child must transition to their new sexual identity to see if they are serious.
Then, puberty blockers are administered
At age 17 or more, they begin hormone therapy

Surgery is not performed until they are an adult and most transgenders do not get surgery
