Yes mostly local and by an individual not a conspiracy.In the past A large electrical company here in Florida has been accused in trying to throw an election by use of fake candidates . There will always be people trying to get an edge in politics.

I think the answer is known. Every investigation has come to the same conclusion, fraud in the US is rare and inconsequential.
