That is some dangerously stupid. There is very little clearance between the subway car and the overhead ceiling because obviously, every additional inch adds to the cost of construction.

Sure, because clearance varies, with there being an AVERAGE clearance and a NOMINAL clearance.

Average clearance is enough to fit a kid in there, but when a contractor comes in there to add a light or run a conduit across the tunnel to power something years later, he may add it so long as it stays within a nominal clearance! And that's the stuff that these kids are getting hit and killed by.
