HC fraud is a huge problem that the US DOJ has also been going after. I listened to a program last night where a medical device manufacturing co. was busted for guaranteeing that you would get a "free" (taxpayer-paid for) device whether you qualified or not.

Powered Wheel Chairs and Medicare Fraud
Witnesses testified on prior authorization requirements for power mobility devices (PMD), such as wheel chairs, in Medicare. A 2011 inspector general’s report found that 80 percent of PMD claims did not meet Medicare criteria and should not have been paid.
