There is no national health insurance program in China. Some cities offer health insurance to residents, most don't. None of the rural people have health insurance, and they have very little access to health care.

As you said, hospitals there ask for payment up front. If you don't have insurance, you'll need cash in hand when you show up. They also don't do appointments. It's take a number and wait. To make matters worse, scalpers show up early and grab up all the numbers and then sell them to people waiting in line.

But wait, there's more! Last year a little girl in Shanghai fell and hit her head and required extensive medical care. Her family could not pay for the very expensive care. However, a crowdfunding group was set up wherein each participant paid about 1/2 a cent and in short order they had raised more than enough to cover the cost. Private individuals pitching in and helping out each other rather than sitting back waiting on a government. This model is becoming a popular new trend in private health insurance in China.
