UPS plane crash at UPS World port Louisville Ky

On fire before it crashed.
 
One of the engines was on fire before it got off the runway, it was headed to Hawaii and was loaded with fuel.

 
Looks like the remains of the aircraft hit a warehouse.

They'll recover what's left of the #1 engine and determine what caused the fire. The MD-11 had trouble getting enough lift on the other two engines (it has three) and fully loaded there was not much room for error.



At least seven people are dead and 11 injured and at least two businesses were hit. There are two people in critical condition and eight with non-life-threatening injuries at UofL hospitals.

Gov. Andy Beshear said he expects those numbers to grow. He also said there were two employees from a nearby auto store who are unaccounted for.

This plane was doomed. The left engine detached from the wing. The wing is on fire in the photo, not the engine.

I learned today that my blue-tooth motorcycle helmet was NOT aboard the doomed plane! It will be arriving tomorrow.

The left #1 engine detached from the wing & right #3 engine was backfiring fireballs indicating compressor stalled. So it only had tail #2 engine giving thrust. This all happened after the point to abort take-off V1 & Rotate had past. There was no chance to recover or avoid hitting buildings & fuel tanks.
 
Preliminary report issued..


Engine detached. Left wing stayed on fire. Plane climbed to 30' and never gained more altitude.

Both lugs show fatigue fractures, particularly on the aft lug...

Different colors here at the break indicate that cracks were present for a while [my interpretation, not from the NTSB report]....

A dried out bearing might increase stresses on these parts. While there seems to be some grease, and no real signs of scratches no the bearing itself, the broken bearing race might indicate that the bearing was bound [me again].

Looks like grease zerk in head of bolt was supposed to pipe grease to small holes in center groove of bearing & out both sides. For some reason grease only came out the foreword race. Only the aft lug looks stress cracked.
