One leg is the healthcare insurance programs.
One leg is the healthcare delivery system.
One leg is the actual health of the people.

Which leg will cause the stool to topple?
 
Another way to put it;
1) A person's care of their health.
2) Medical Care for when personal care fails or couldn't work.
3) Medical Insurance to help pay for the Medical Care.

I know mine are a bit more wordy, but maybe more precise.

All three could cause "the stool" to topple, however;
I'd say it mainly begins with "1) A person's care of their health." since if one takes good care of their health the other two are not needed as much, if at all.

Sadly, that's the first leg to buckle taking the others with it.

Notice that leg #3 is being soundly and deliberately ignored by the other legs. Possibly the greatest conspiracy in history, bringing a nation down without firing a shot.

What will be done about it? No penalty for brokers filing false applications?

Among them:

7,000+ SSNs belonged to people who died before they were supposedly enrolled, and
19,000+ matched SSNs used in applications with entirely different names and birthdates, suggesting synthetic identity fraud.
