There is nothing automatic about accumulating wealth if you are super-intelligent. Many fortunes are made on ideas that are stupid. Facebook? Netflix? Amazon - a failed attempt to sell books?
Just because you are intelligent doesn't mean you can predict what other people will do - which is how you get very rich.
Even if you decided to pursue medicine, you cannot outpace the prior art, and humans are infinitely variable, so you couldn't save every patient.
Say you decided to make a killing in the stock market...it would take luck. Money managers already have a near infinite amount of information at their fingertips, but it is not possible to KNOW where the market is going.
