That looks painful.
It is an allusion to and suggestive of the (otherwise) "forbidden fruit" of the garden of Eden

No doubt... they picked a big, fat, swollen eye to photo shop that apple in there, giving it that added illusion.
It means, "An apple a day keeps the doctor away"
So if one was to eat and apple, it would keep you, Dr. xotoxi, away from us all.
