What is the definition of "private" healthcare according to the study?

Because currently we do not have anything close to free market healthcare. We have insurance companies forced to provide coverage for everything ranging from routine doctor visits to plastic surgery. Only 10% of our hospitals are private, for-profit institutions. Often times "privatize" does not mean "free markets." It means big business and big government together.
