Actually ... your first line is incorrect, male to female can still orgasm as well. The nerve endings can now be protected, nothing is "chopped off", both are just inversions of the genitals. Female to male does not require breasts to be removed either, the hormonal therapy takes care of that, it can be done it major surgery is often avoided unless needed and most patients will respond to the the hormonal changes quite well, liver damage being the biggest risk since they are the same as steroids.
