The bottom line is that we can’t have fairly priced insurance for the healthy and sick alike without the broad participation that is guaranteed by the mandate. The mandate is the spinach we have to eat to get the dessert that is fairly priced insurance coverage.

ACA demands that insurance companies spend 80% of their premiums on patient care. Goodbye to multi million dollar salaries to their CEOs.

Completely different. States are within their mandate as a local government, the federal government is not. It's not rocket science. If the people of a particular state want a health care system, they should have it. It has jack shit to do with the federal government. It is cheaper, more efficient and more cost effective for states to run their own affairs.
