Direct federal administration of states insurance exchanges will induce sooner rather than later transformation to universal federal healthcare insurance. Not availing themselves of the generous federal benefits offered for expanding Medicaid will only serve to increase those states healthcare costs and provide their citizens with lesser healthcare.

The provisions of the Affordable Care Act, (correctly described as the Re-admissions Reduction Program), does not penalize hospitals for readmitting patients. It does penalize hospitals practicing policies that result in statistically clear increased readmissions.
Such hospitals attempted (and occasionally succeeded) to reduce their own costs by passing the consequences of their policies upon others.
Patients received sub-standard healthcare and along with their families they additionally suffered lesser qualities of life due to those hospitals sub-standard practices. Everyone (which certainly includes USA taxpayers), directly or indirectly paying for the additional expenses due to those hospitals practices of false economy, have in effect had their pockets picked.
The Re-admissions Reduction Program is not socialist or communist, or capitalist or liberal or conservative policy; its sensible financial policy.

USAs hospitals span the entire ownership spectrum; there are corporate, university, charity, partnership and government owned clinics and hospitals. A great proportion of health care is provided by those who financially profit from their own enterprises. Thats factual and that's capitalism.
