One of the biggest reasons that insurance coverage in many states is prohibitively expensive are the must-cover mandates for things like drug/alcohol rehab, chiropractic, minor out-patient procedures, etcetera.

It's somewhat surprising that some of the more observant here haven't caught on to this aspect of the recent kerfuffle.
