American Cancer Society now favours regular mammograms from 45
Tuesday 20th October, 2015 | WASHINGTON - In a major change of stance, the American Cancer Society has recommended that women at average risk of breast cancer should begin annual mammograms at age 45 -- five years later than it had previously recommended.
In new guidelines released Tuesday, the society said it reached the conclusion after carefully weighing both the risks and benefits of mammograms for younger women, who face a lower risk of breast cancer than older women. For women with an average risk of breast cancer, which signifies most women, should begin yearly mammograms at age 45, state the new guidelines. By age 55, which is five years over the age recommended by a federal government task force six years back, women can transition to being screened every other year as the risk of breast cancers and its growth become slower after menopause, making it safe for women to be checked less often. The practice of having regular mammograms should continue for as long as a woman is in good health.

Deviating from the earlier recommendations, the new guidelines state that breast exams, either from a medical provider or self-exams, are no longer recommended as they have not been shown to save lives. The new guidelines reflect the growing recognition that mammograms can do harm, as well as good, said Richard Wender, chief cancer control officer at the American Cancer Society. "The biggest evolution has been not in the American Cancer Society, but in the science of cancer screening and the evolution of health care in general," Wender said. The new approach is based on the biology of breast cancer and a woman's changing risk as she ages, Wender said. "We know that debates will continue about the age to start mammography," said Wender. "This guideline makes it so clear that all women by age 45 should begin screening that's when the benefits substantially outweigh the harms."

But once a woman turns 55, said Elizabeth T.H. Fontham, member of the Guideline Development Group, screening every other year preserves most of the benefit of screening every year with fewer risks. The society picked 45 years as the start date for screening as it is the age when a woman's risk of breast cancer begins to spike. Prior to 45, the problem caused by mammograms is often "over diagnosis" wherein screening tests detect slow-growing cancers that are destined never to threaten a woman's life. As it is difficult to distinguish which breast tumors are essentially harmless, they end up treating all of them, leading to unnecessary surgery, radiation or other treatments. The guidelines state that while the goal of screening mammograms is to find breast cancer early, when treatment is more likely to be successful, but mammograms aren't perfect, and they do have risks.
