Israeli-Developed 'Artificial Pancreas’ Tested

Apparently for the first time in the world, a recent research study on children with type 1 (insulin-dependent, juvenile- onset) diabetes aimed at creating an effective artificial pancreas to produce insulin was held not in a hospital but in a hotel.

The clinical trial was conducted by specialists at the Schneider Children’s Medical Center (SCMC) at the Kibbutz Ma’aleh Hahamisha Hotel outside Jerusalem. The prospective, crossover study, in which the whole group receive the treatment but at different times, was part of the three-day DREAM (Diabetes wireless Artificial pancreas consortium) Camp for Children with Diabetes.

It consisted of 18 kids aged 12 to 15 years; nine of them were were connected to the artificial pancreas system on the first night of the camp, while eight were connected on the second night. The benefit of being in a hotel rather than a hospital ward was that the more pleasant, non-threatening environment made the results more accurate.

The MD-Logic Artificial Pancreas, developed at the SCMS’s Institute for Endocrinology and Diabetes, consists of an off-the-shelf subcutaneous glucose sensor that monitors glucose levels, and an insulin pump. The sensor and pump are connected to a computer that programs the information and stipulates the amount of insulin that should be released to the body in order to maintain blood glucose balance.

This innovation “closes the loop” between the sensor and the pump and relieves the patients with diabetes from the daily burden of dealing with their diabetes and has the potential to significantly improve the quality of life of patients with diabetes.

The experimental artificial pancreas is aimed at providing a real solution for patients with type 1 diabetes, which usually begins in early childhood. The research is being carried out at the Shaffer Institute for Endocrinology and Diabetes at the Petah Tikva hospital’s National Center of Childhood Diabetes. The project is headed by institute director Prof. Moshe Phillip and Dr. Revital.

Following the successful trial, Phillip said: “I am proud to head and be a part of the pioneering team working for a number of years to find a technological solution that will significantly improve the lives of millions of diabetics around the world. The MD-Logic Artificial Pancreas and the DREAM Projects will enable patients to be relieved of the continuous need to check glucose levels and inject insulin, while ensuring long-term balance which significantly reduces the chance of complications from diabetes. The entire staff in the Institute is excited to be part of this important historic project.”

“This is a significant landmark in research,” he added. “This is, to the best of our knowledge, the first time in the world that such a trial project has been taken outside the hospital, illustrating a normal life for youngsters with diabetes, while using an automatic insulin delivery system during the night.” The overall success of the research represents genuine good news to millions of patients with diabetes in Israel and the world, he said.

20 February 2012 - Doctors want to improve the prognosis of this aggressive cancer

Scientists say they may have found a new weapon against pancreatic cancer after promising early trial results of an experimental drug combination. Giving the chemotherapy agent gemcitabine with an experimental drug called MRK003 sets off a chain of events that ultimately kills cancer cells, studies in mice show. Patients are now testing the treatment to see if it will work for them. The Cancer Research UK-funded trials are being carried out in Cambridge.

Father-of-two Richard Griffiths, 41, from Coventry, has been on the trial since being diagnosed with pancreatic cancer in May 2011. "After six cycles of treatment, a scan showed the tumours had reduced and so I have continued with the treatment," he said. "The trial gives you hope - I really feel I can do this with the science behind me."
Aggressive cancer

Cancer Research UK says it is prioritising research into pancreatic cancer because the survival rate still remains dismally low. About 8,000 people in the UK are diagnosed with pancreatic cancer each year, and the disease is the fifth most common cause of cancer death in the UK. Survival rates are very low in relation to other cancers, and the length of time between diagnosis and death is typically short, usually less than six months. The most recent data for England show that about 16% of patients survive the disease beyond 12 months after diagnosis - prompting the need for new treatments.

Professor Duncan Jodrell, who is leading the trials at the University of Cambridge, said: "We're delighted that the results of this important research are now being evaluated in a clinical trial, to test whether this might be a new treatment approach for patients with pancreatic cancer, although it will be some time before we're able to say how successful this will be in patients." In total, about 60 patients with advanced pancreatic cancer will be recruited for the first Phase I/II clinical trial.

Potential Breakthrough in Pancreatic Cancer Treatment Reported
March 20, 2012 - Overall survival increases by 70% in mice

Scientists in Seattle are reporting a potential breakthrough in the treatment of pancreatic cancer, a disease which stubbornly resists most therapies. Pancreatic cancer tumors are resistent to chemotherapy partly because they form a biological barrier around themselves. Researchers at the Fred Hutchinson Cancer Research Center believe they've found a way to break that barrier down. "Pancreatic cancer actually has the highest one-year and five-year mortalities of any cancer," says Sunil Hingorani, senior author of the study published in Cancer Cell.

That high fatality rate has been something of a puzzle for researchers who study pancreatic cancer. Powerful chemotherapy drugs that kill off pancreatic cancer cells in the laboratory are next to useless in living cancer patients. Scientists have observed how tumors in the pancreas, in effect, wall themselves off with a collagen material similar to scar tissue. That exerts pressure on the blood vessels that supply the tumor, collapsing many of them and restricting not just the flow of blood but also of chemotherapy drugs. "And so in this way, the tumor actually isolates itself completely from the circulation, and when we try to give drugs through the vein, they essentially bypass the tumor and instead go everywhere else," Hingorani says.

So the problem may not be that the chemotherapy medicine doesn't work, but that it simply may not be able to reach its target. To find out, Hingorani and his colleagues used mice genetically engineered to stand in for human pancreatic cancer patients. They treated them with the standard chemotherapy drug, gemcitabine, plus the enzyme PEGPH20, which is designed to open up the collapsed blood vessels that supply the tumor. "We found that in essentially every animal that we tested, we saw the tumors shrink or at least stop growing," he said. "And we found that overall survival increased by about 70 percent — so not quite doubling, but just about."

Of course, what works on mice doesn't necessarily work on people, so the researchers are beginning human trials. Hingorani says he is optimistic, but cautions that it will be a year or two before results are available from the tests with people. However, if all works as the researchers hope, not only would the chemotherapy be more effective, there may be fewer nasty side effects. That's because with more of the medicine actually reaching the tumor, doctors may be able to use less of the powerful chemotherapy drugs.
