When you become injured or ill you will be treated and billed. When you don't pay the bill interest will accrue and eventually your obligation will be turned over to a collection agency, your credit rating will drop and you will be sued; a civil judgment will be ordered and enforced. Wages can be collected, income tax returns taken and life will change because of your own stupidity.

If you never need a doctor there will be no penalty; in that case you will live happily and in good health as a scofflaw.
