Personally, I think they need psychiatric help.

And maybe society has no right to get these people help, BUT society does have the right to decide not to go along with their fantasies by making these people "mainstream".

Not only that but the doctors who make them sign waivers that say "you realize this surgery/drug therapy will never actually make you what you want to be", then continue to do unnecessary and highly risky invasive amputations of the otherwise healthy genital/urinary system, should be stripped of their licenses and thrown in prison for 10-20 years.
