For starters, don't go around forcing people to buy stuff and pretend it's a free market.

One problem is those who believe that allowing customers to shop across state lines (because Blue Cross in one state has different rules from Blue Cross in another state) is a Violation of States' Rights!!!!!

No, it's a (rare) proper application of the commerce clause. The federal power to regulate interstate trade was specifically designed to prevent states from erecting trade barriers.

You also need to make the policies more a la carte. I think we are well past having a baby at my house. More concerned about major medical issues and would pay out of pocket for basics, if my overall premiums were better. This would allow me to actually use the coverage.

Agreed. Right now Hopeycare is requiring 80 year old women to get covered for maternity.
