So who gets to decide who lives and who dies? The patient, his family, the medical community, the government, Sarah Palin, you?

It's up to the patient. If he has the money to pay for the treatment, then he lives.

That's the point......the patient can fork up their own money for a low probability (of success) effort.

If there was a good chance of an extension of life with good quality....it seems more reasonable.

I've seen far to many situations where people are getting expensive treatment despite the fact they are pretty much gone (and in some cases just want to move on....it was family that insisted on the treatments).
