Fact: page 14: "And some individuals who are subject to the mandate are nonetheless exempt from the penalty -- for example, those with income below a certain threshold and Indian tribes."

page 13: "The individual mandate requires most Americans to maintain "minimum essential' health insurance coverage. The mandate does not apply to some individuals, such as prisoners and undocumented aliens. Many individuals will receive the required coverage through their employer, or from a government program such as Medicaid or Medicare. But for individuals who are not exempt and do not receive health insurance through a third party, the means of satisfying the requirement is to purchase insurance through a private company."
 
Of course there will be scofflaws who think they can get away for year, maybe decades, and not pay for health care. Some day they will need health insurance and that day may come sooner than later. Accidents happen and when they do and an emergency room visit is required the cost will shock them.

When the bill is not paid, it will go to a collection agency and interest will accrue. One day their tax return will be intercepted and their desire to buy a car will be denied for their credit rating is much too low. Someday a civil court will render a judgment and the debt, growing at the legal rate of interest will include court costs, attorney fees, filing fees and such, about that time the decision not to buy medical insurance will seem to have been a pretty stupid one.
